

Leveraging data analytics to improve health outcomes and strengthen patient engagement.
Patient engagement is critical following major cardiac surgery, as patients present with a wide range of health conditions and recovery needs in the months after discharge. Collecting patient data and maintaining consistent follow-up can be time-consuming and complex. Without a unified view of patient health trends, clinicians struggle to monitor recovery effectively, making it difficult to provide timely guidance and deliver consistently high-quality care.
We implemented a patient engagement platform that included a mobile application (MICArdiac) and a data and analytics platform to support the WARC research project. The platform brings together patient-reported data, including secure messaging, along with data from external sources and connected health devices, such as step count, blood pressure, and heart rate, into one secure place. On top of this, we developed dashboards and predictive models to identify at-risk patients, support care decisions, and provide real-time alerts to clinicians.
